     service (document #469) at 1-800-458-5231 extention 7091 (USA & Canada), 
     
     International Telephone number is 1-301-519-0459 fax service  extention 
     7091 (document number 469), OR regular FAX # 1-301-519-6616 to request a 
     copy of the document to be sent by mail/post. 
     
     According to CDC in Atlanta, GA, the document is in the process of 
     being revised, as of December 1997 CDC has no anticipated date of when 
     the document will be completed/available.
