Dear Forum Members:

I think there are areas of HCC that need to be strengthened and other that need to be
launched. The draft programme that we have on hand must to conquer these objectives. In
my opinion, there are issues that should be addressed by this programme. For example:

- I was wondering in which places with a HCC programs exist a deep understanding of the
needs of PLHA's and what are the responses to their needs. There are just few studies
about indicators on quality of life of PLHA's identified by themselves. Quality of life is
an on going process that deserves to be studied periodically.

In the last HCC Conference in Chiang Mai, Dr. Laksami Suebsaeng gave me the report study
"PHA Indicators of Well Being". I think this an important document that could help to
reproduce studies elsewhere.

- Is there an available framework of HCC, which can be used in all places or regions? The
response is yes. Recently Dr. Hedwig Goede from the Care and Support Unit from WHO sent
the publication "Community Home-Based Care in Resource-Limited Settings. I think people
interested in HCC issues must to be encouraged to review this document. This document must
be used widely in developing countries.

- Guidelines for HCC programmes design and implementation are useful, but those need to be
adapted to the culture of each region. Please pass lessons learned about this topic.

- Lessons from community caregivers who are from CBO (Community Based Organisation) are
valuable, and there is so little attention to register these efforts. HCC Conference is an
excellent effort to bring the local, regional and global attention to this matter.

- Is there comunication among different examples of HCC in the world for example those
from CBO, religious groups, PLHA's networks and government (hospitals and other health
settings)? I remember the paper from Dr. Eric Van Praag about this issue.

- Is there a REAL connection among different approaches:
* A comprehensive health approach; * A person-centred approach and * Goals centred on
access to treatment? Or, are they isolated events? Is the burden of the financial
resources from the budgets allocated to health and social security the obstacle to put in
the scenario of the performance of the care of PLHA's?

- Is there competence between care for PLHA's and other sick people or terminally ill
people in each health setting?
- Is the Home and Community Care for PLHA's issue influencing local, regional and global
public policy? It is urgent to summarize the efforts from advocates and fighters for
caring of the PLHA's around the world?
- Is the HCC for PLHA's a matter of human rights? The right to the health, services of
quality, among others rights?
- Are the governments passing from the rhetoric to the real action in each country?

I think the forthcoming Home and Community Care to be held in Dakar, Senegal is a
wonderful opportunity to discuss relevant topics, exchange ideas and to share practical
information about the questions above. I am sure that the task to make the programme is a
wonderful opportunity to contribute with relevant topics, exchanges point of views, know
lessons learned in the field and to find realistic ways to promote a positive environment
to launched HCC programmes in those place where these do not exist.
