If you use the HIV/AIDS and Sexual and Reproductive Health Integration Web site (http://www.hivandsrh.org <http://www.hivandsrh.org/> ), we invite you to complete a short on-line survey. 

As a thank-you gift for participating, after you submit your answers, if you submit contact information, you will be entered into a drawing for your choice of prizes. Five respondents will be randomly chosen to receive a free copy of either: (1) Family Planning: A Global Handbook for Providers (English or Spanish)--Published by the INFO Project at the Johns Hopkins Bloomberg School of Public Health, the book offers clinic-based health care professionals in developing countries the latest guidance on providing contraceptive methods; or (2) the World Health Organization's Medical Eligibility Criteria Wheel for Contraceptive Use. This easy-to-use job aid will help providers quickly to identify Medical Eligibility Criteria relevant to their clients.
